Full text: 

O Domine Iesu Christe, per illam amaritudinem quam sustinuisti in cruce et maxime in illa hora qua anima tua sanctissima egressa est de corpore sanctissimo, deprecor te, miserere animae meae in egressu suo et perduc eam in sinum Abrahae.

English translation: 

O Lord Jesus Christ, for the sake of that bitterness that thou didst sustain for me, hanging on the cross, above all when thy most noble soul departed from thy most holy body, I pray you, have mercy on my soul in its departure and guide it to Abraham.
